  3. Your IM-Home Self-Training • How did it go? • Are you comfortable with the eClinic? • Did you modify your training plan?

  4. When to adjust the training • Be able to keep your score under 100 ms before increasing reps • Example: If you have a 250 ms on both hands, set their program for 50 to 100 reps and keep it there until their scores are below 100 ms. • Tip: Demo hand over hand to parent or caregiverto help them

When to adjust the training • Gradually increase the reps as you are able to maintain scores consistently under 100 ms • As the scores drop lower add longer reps to EACH TASK • When able to do tasks with longer reps (starting with the 500 reps tasks) make sure they can maintain their scores in the average to above average range

  6. When to adjust the training • An off session or two can be expected at some point in the training • Don’t change plan just yet • Some clients have a few poor scores right before they make a big gain in their timing • If lack of progress persists, it is time to reassess

Let’s face it… • IM was meant to be offered as a home program. • It’s not realistic to expect a family to come to the clinic as much as IM needs to be done* • But your job is to provide your clients with the best care possible. • *To make permanent neurological changes in the brain thousands of repetitions need to be done. …that’s where IM-Home comes into play

  10. Models of Use • Just because the client isn’t physically seeing you, doesn’t mean he/she shouldn’t be charged for your time • Hybrid • Mix of In-Clinic & Home allows patient to bill insurance part of the time • Virtual • Great for those with travel issues or a discharge program

How can I charge my client That much? • A plumber spends 10 minutes fixing a pipe and hands the woman a bill for $100. • The woman is shocked by the bill and asks why it’s so much, it only took him 10 minutes. • The plumber replies, why yes, but it took me 20 years to know exactly where to hit that pipe so it doesn’t blow-up and flood your house!   16. Client Selection • IM-Home IS NOT FOR…Clients who: • Have severe disabilities and need professional guidance • Unable to commit to training 45 min, 5 days a week • Are looking for an instant fix (ie. Medication)

IM-Home is perfect for clients…. • Who can’t make it to the clinic at least 3 times per week. • Who need to focus on other modalities during their in-clinic time, but still would benefit from IM. • Who have completed the IM training in the clinic and would benefit doing IM to tackle future developmental milestones. • Who have a degenerative condition.

Office to Home Consultation • When ready to shift to home set up a meeting with the parents/caregivers • Review progress to date • Review why you think the client is ready to shift to home • Review what schedule might be best for the client and the family • Help parents complete the IM home schedule before they end the meeting.

Shifting IM from office to home • Help identify the best space for doing IM • Help parents identify the best schedule for the shift to home: For example • First week-2 sessions in the office, 1 at home • Second week- 1 session office, 2 at home • Third week-All IM sessions at home with ½ hr consult from IM provider
