What is TC CAMP? • Partnership between Eastern Health, NARI and the Department of Health • Commitment to continuous improvement – right care, right time and right place • Bed based specialist service for clients with a severe cognitive impairment • Six month pilot seeking to identify best practice approach for target group

Key findings of the TC CAMP Evaluation • 11 families consented to take part in the evaluation from 23 admissions • Av age 81 (range 68-89) • Gender 71% women • LOS average 46.3 days (range 8-89) • All clients were high or super high falls risk in acute • MBI 64.0 admission 68.2 discharge • Discharge destination • 8 SCF high - 1 SCF low • 1 palliative care - 1 psycho-geriatric
Key findings of the TC CAMP Evaluation cont. • Consistency between program design and implementation • Provision of Allied Health Key to Me • TCP facility found it useful if they arrived with the client - not always possible • Quotes from subsequent facilities: “finally a facility that got it right” “often we only find out the things we’d like to know about a person in their eulogy”
