The port terminal request us to provide rectified docs/action relative to PSCI deficiencies. We have sent attached mail to port terminal but still lack of b/m request report. Would you please urgently provide such report to make vessel smooth call at Rotterdam. =qte= 1. Preventative actions to avoid re-occurrence of recorded deficiencies. 2. there was an engine break down (casualty report) on 8.1.09 - please request owner/operator to forward incident report and follow up actions taken =unqte=
