Partnership from a RAAM perspective Partnership: Continued networking Agreement to work together to advance mutual interests • I.e., the patient/client we are serving Partnership may require sharing relevant information Knowing what information/service each partner needs in order to create that substantial and strong partnership

  4. What do you need to know? How do you know that this is the right referral? Know the client Know the resources/agencies: What services do they provide? How will this benefit the client? Where and when to make the referral or connection? How is the referral made? E-mail, phone call, referral form? What method of referral works best for your partnership? Does the referral process work for the client? Is this partnership working? Is there need for ongoing meetings or redefining the relationship? Are all needs being met? What (or who) has changed?

  5. Are the right players at the table? What are your population’s needs? Has the population changed? Have there been any changes in the partner? Are you connected to the right person? Have any services been added or taken away? How was the partnership created? Is there a gap in the partnership? Is there a need to clarify the relationship through formal or informal meetings/agreements?

  6. Feedback from partners Follow up with coworkers, partners, and clients to make sure the partnership is working Are clients getting what they need? Is anyone frustrated? Are the services as advertised? Are changes needed? How can the changes be made?
