PDSA P = Plan Define system, Assess the current situation, Analyze causes D = Do Try out the improvement theory S = Study Study the results A = Act Standardize improvements, plan for continuous improvements
Identify the problem Determine the root cause.
Root Cause One definition – "The cause of a problem which, if adequately addressed, will prevent a recurrence of that problem".
PDSA Identify the problem. Next ask, “Why?” By asking the question "why” five times, the root cause of a problem is often identified as a procedural, or management, shortcoming. Addressing these root causes often requires a change of thinking and some pain and effort, but the results will be much longer-lasting and higher-value than correcting individual failures.
Finding W. Edwards Deming's estimated that 94 to 97 percent of our problems stem from the system with the remaining problems caused by human error or poor judgment.
