Who starts Them • All patients admitted with a primary diagnosis that we have a pathway for should be on that pathway. • The pathway is to be started by the first unit that has contact with the patient and it is to be entered as an orderable under Cerner (ER, Pre-admit or Woman and Child Unit) • Start the pathway at the appropriate time frame If you receive a patient to your unit and the pathway has not been started and enter into Cerner
What do I do with Them? • Have pathway materials you need ready with admission package when a patient is admitted • If you start the pathway in ER or pre-admit complete the documentation and forward the package to the receiving unit for completion of the pathway • Clinical Pathway remains as part of the patients chart and sent to medical records except for the Variance Form.
Variance Record/Discharge Summary • Once the patient is Discharged the Variance Record/Discharge Summary form is sent to the Evidence-Based Care Program via inter-hospital mail pre-addressed envelopes are available for each unit and will be returned to you for reuse. (Envelope mailing system)
