Poplar NCB • Target area comprises Bromley by Bow and Mile End East wards, both of which are amongst the 2% most deprived wards nationally. • 42% of the population are Bangladeshi • Poor health indicators - higher rates than Tower Hamlets average for Diabetes (6.8% v 4.6%) and related co-morbidities • There are approximately 1,880 patients diagnosed with diabetes in the target area (total population 27,689) • Highest hospital admission rates and hospital costs per head of all areas in Tower Hamlets - 522 hospital admissions related to Diabetes each year

4. Poplar NCB Summary The project will combine the delivery of effective Primary Healthcare Packages with community engagement in healthy living and health awareness. Activities • Add value to the current diabetes clinical care packages by incorporating areas that impact on the wider determinants of health such as: employment, education, housing and welfare benefits • Community engagement and outreach through Healthy living activities; health focused workshops; volunteer community health advocates; • Training and employment opportunities offered at Burdett centre
