Section 504 Checklist of procedures to follow to ensure compliance with State Section 504 Laws/Code: • Notify parents or guardians of proposal to assess student for 504 eligibility (§504). • Inform parents or guardians of their rights under Section 504. • Obtain parent or guardian permission to assess the student. • Administer measures using only individuals/professionals who are trained in assessment to evaluate students for suspected disability. • Ensure eligibility determination is made by a committee that includes persons knowledgeable about the related disability. • Provide needed instruction and/or accommodations.
