Rehabilitation assessment For determining any urgent and ongoing medical management, vocational and psychosocial needs, the approved rehabilitation service provider undertakes a rehabilitation assessment. You will fill out a lifestyle satisfaction indicators form as part of the rehabilitation assessment. They also include personal or nursing care providers and services which assist in altering residences and work or training environments. A wide range of continence products to eligible members of the veteran community is provided by the DVA through the DVA rehabilitation programs. If the applicant is an entitled war widow(er) or a veteran, they can access support if they hold a Gold Card. It is eligible for treatment of all conditions whether or not they are related to war services.
