Growth Assessment Hemoglobin/Hematocrit • A “failed” growth assessment is a BMI at: • 95% or higher • 4% or below • A “failed” event is a: • Hemoglobin below 11.0 • Hematocrit below 34% Diet History A “failed” event is when question 2e is answered “yes – child eats non-food items” Also, notify dietitian if child has a food allergy, food intolerance, special dietary needs, or avoids foods for religious reasons ASQ:SE Notify dietitian of nutrition issues such as stuffing food in mouth, inadequate food intake, or other parental concerns regarding child’s diet Doctor’s Note & Physical Notify dietitian of diagnosed condition: Obesity Anemia Food allergy or intolerance Megan Smith, MS, RD, LD
