Who is Eligible to Enroll? • Employees – Regular full-time employees who average 20 or more hours a week for two consecutive quarters • Dependents are defined as your: • Spouse (as legally recognized by the State of Tennessee) while not divorced or legally separated • Dependent Children from birth to age 26, married or unmarried as long as he/she: • Is your child by birth, adoption, legal guardianship or your stepchild whose primary residence is with you or your spouse; and • The child is not eligible for his or her own employer-sponsored health plan. • You may also cover your children when Qualified Medical Child Support Order has been issued.
Enrolling in Coverage • You decide today what benefits you want to enroll in and submit your form to Metro Human Resources. • Coverage is effective the first day of the month after one full calendar month of employment. • Insurance premiums are deducted one pay period in advance, through convenient payroll deductions each pay period. • What if I do not enroll in coverage? • If you do not complete and submit an Enrollment Form, you will automatically be enrolled in single coverage with BCBS PPO and Delta Dental Premier Plan. • Opting Out of Coverage • If you have other medical insurance coverage – either through a spouse’s or parent’s plan – you may opt out of Metro’s insurance by providing documentation of the other coverage. • You may opt into Metro’s coverage at Annual Enrollment or within 60 days of an eligible change in status.
Making Changes in your Coverage and/or Dependents • You can change your coverage during the year only: • If you have an eligible change in status or • During Annual Enrollment each Fall • Examples of eligible changes in status include: • Marriage, legal separation or divorce • Birth, adoption or legal guardianship • Change in spouse’s insured status or job • Change in child’s eligibility • Death of a spouse or dependent You MUST notify Metro Human Resources within 60 days of your eligible change in status; otherwise, you will NOT be able to make changes to your coverage or dependents until the next Annual Enrollment.

BCBS PPO • Plan pays 80% of covered medical services – you pay 20% coinsurance after required copays. • $20 copay for primary care physician office visits • $30 copay for specialist office visits • $100 copay for emergency room visits • Preventive Care Benefit • Children age 6 and younger covered at 80% • Age 7 and older covered at 100% up to $750, then at 80% • Pharmacy Benefits • $10 copay for generic drugs • $30 copay for brand-name drugs
CIGNA Choice Fund • HRA Fund – Metro puts $1,100 (single) or $2,200 (family) into your Fund each year. Fund pays medical and pharmacy expenses before you pay anything out of your pocket. • Once Fund is exhausted, you pay your share of the deductible - $450 (single) or $900 (family). • After the deductible has been paid, you will pay coinsurance of 10% for your medical expenses and generic drugs and 30% of brand-name drug costs. Once you’ve met the annual out-of-pocket max, you have 100% coverage. • Preventive Care is covered at 100% by the plan and is not deducted from your Fund.
CIGNA Choice Fund Incentives Your Fund will be increased if you take the online health risk assessment or if you qualify and participate in the following incentive programs: • Health Risk Assessment • $100 per person upon completion ($200 maximum per family) • Available to employees, spouses and dependents age 18 or older. • This questionnaire is short, simple and easy to use, and helps CIGNA provide you with a holistic view into your health risks and provides a personalized health profile. • Disease Management for Cardiac, Diabetes and Chronic Obstructive Pulmonary Disease (COPD) • $100 per person ($200 maximum per family up to 2 family members can qualify for 1 program/year). • Active participation is defined as engaging in three (3) telephone sessions with a CIGNA Well Aware nurse.
CIGNA Choice Fund Incentives • Tobacco Cessation Program • $50 per person ($200 maximum per family). • Active participation is defined as engaging in two (2) telephone sessions with a CIGNA coach within the 12 month duration of the program. • Healthy Pregnancies, Healthy Babies • $150 if you enrolled by the end of your first trimester or • $75 if you enrolled by the end of your second trimester. • Participation in a program designed to help you and your baby stay healthy during your pregnancy. This program encourages you to get prenatal care early in your pregnancy. When you complete the program and after your baby is born, you will receive the incentive.

General Hospital Incentive Program • General Hospital is offering an incentive program to Metro Employees, pensioners and their dependents where they may access care through General Hospital or Meharry Medical Group without incurring copays or deductibles. • Your medical insurance will be billed but you are not responsible for any copays or your portion of the deductible. • Services include primary care, specialists, in-patient care, radiology (x-ray, mammograms, MRI), lab work, physical and occupational therapy. • For more information, contact General Hospital at 341-4YOU.
Retirement Pension – General Government • Metro pays 100% of pension plan benefits – employees do not contribute. • Normal unreduced pension benefits begin after age 60 once your age plus years of service equal 85 points. Rule of 85 = Age + Years of Service • Early reduced pension benefits may begin at age 50 once you have 10 years of service.
Retirement Pension – Police & Fire • Metro pays 100% of pension plan benefits – employees do not contribute. • Normal unreduced pension benefits begin after age 53 once your age plus years of service equal 75 points. Rule of 75 = Age + Years of Service • Early reduced pension benefits may begin at age 45 once you have 10 years of service.
Disability Pension • Metro offers two types of disability pension benefits – 100% paid for by Metro with no employee contribution. • Medical Disability – must have 10 years of service • In-Line of Duty (IOD) – immediate coverage for work-related injuries • Monthly pension benefit is 50% of your last 12 months earnings • If you have questions about disability benefits, contact Metro Human Resources at 862-6700.
Connection of Service • If you worked for Metro Nashville Government before, left and have now returned, you may be able to connect your prior service after one year of continuous service in your new Metro job. • By connecting your prior Metro service to your current service, you’ll receive a greater service pension benefit at your retirement. • To connect your service, please contact Human Resources at 862-6700.
Basic Life Insurance • $50,000 of basic life insurance provided by Metro at no cost to you. • Metro provides Accidental Death & Dismemberment (AD&D) benefits if you suffer certain injuries or if you die in an accident – the amount of this benefit depends on the type of injury. • Basic and AD&D benefits are reduced to 65% on January 1 following your 65th birthday. • You will need to complete a beneficiary form today. • Prudential administers Metro’s life insurance program.

Optional Benefits • Optional Benefits are available to provide you with a measure of financial protection and security. • You pay the full cost of the optional benefits at group rates. Payments are made through payroll deduction (with the exception of Long-Term Care which you pay directly to the carrier). • Reenrollment in optional benefits is automatic each year except for the Flexible Spending Accounts.
Coverage without Proof of Good Health • As a new employee enrolling now, you may enroll in Supplemental Life Insurance, Dependent Life Insurance, Long-Term Disability and Long-Term Care without providing proof of good health. • If you have a known health condition that could possibly preclude you from being approved by the insurance company, you should strongly consider enrolling now. • If you decide to wait and enroll in the future, you will be required to provide proof of good health. • If you enroll now and drop coverage later, you must show proof of good health to re-enroll.
Supplemental Life & Dependent Life Insurance • Choosing Supplemental Life and Dependent Life is a personal choice. Life insurance provides your family or dependents a measure of financial security in the event of a death. • When considering the option, you should consider your: • Marital status • Dependent’s ages and requirements • Other income sources • Debts • Savings • Retirement benefits • Supplemental Life & Dependent Life is offered through Prudential.
Supplemental Life & Dependent Life Insurance • Supplemental Life • Provides a lump sum payout to your beneficiary in the event of your death. • You may purchase coverage in increments of $10,000 up to a maximum of $200,000. • Dependent Life • Pays you a lump sum benefit of $10,000 in the event of your spouse’s death and $5,000 for the death of a dependent child. • You must be enrolled in Supplemental Life to purchase Dependent Life insurance. • Monthly premium is $2.18 regardless of the number of dependents covered.
Short-Term Disability • Short-term disability benefits replace 60% of your Metro salary if you become disabled and cannot work because of an illness or injury. • Benefits begin after a 7-day waiting period and may continue for up to 180 days. • If you do not enroll within 60 days of first becoming eligible, you will be subject to a late enrollment penalty where the waiting period would be extended from 7 days to 60 days for any condition other than an accidental injury. • In certain circumstances, pre-existing medical conditions may exclude you from being eligible for benefits for the first 12 months of employment. Contact Metro’s carrier, The Standard, before you enrolling if you have a pre-existing condition.
Long-Term Disability • Long-term disability benefits replace 50% of your Metro salary if you become disabled and cannot work because of an illness or injury. • Benefits begin after 180 days of continuous disability. • If you do not enroll within 60 days of first becoming eligible, you will be subject to the late enrollment penalty. • In certain circumstances, pre-existing medical conditions may exclude you from being eligible for benefits for the first 12 months of employment. Contact Metro’s carrier, The Standard, before you enrolling if you have a pre-existing condition.
Prudential Long-Term Care Insurance • Long-Term Care coverage provides help or supervision needed for someone with severe cognitive impairment or the inability to perform two of the normal activities of daily living such as bathing, dressing, eating, toileting, transferring and continence. • Coverage includes: nursing homes, assisted living, home health, respite care, adult day care, informal care and hospice care. • Do not confuse Long-Term Care coverage with Long-Term Disability coverage which is designed to cover employees during injuries and illnesses that prevent them from working.
Prudential Long-Term Care Insurance • If you enroll when first eligible, you are not required to provide proof of good health. • With proof of good health, you may also cover your spouse, your children over age 18, your parents and your spouse’s parents. • Premiums are direct billed by Prudential. • To get premium information or enroll, contact Prudential directly at (800) 732-0416.
Flexible Spending Accounts • Flexible Spending Accounts (FSA) allow you to set aside a tax-free portion of your salary to reimburse yourself for many types of care expenses not covered by insurance. • Metro offers two tax-saving options under the FSA program: • Health Care Account – includes most health care expenses that are not covered by your medical, dental and vision plans or out-of-pocket expenses you pay such as deductibles, copays, prescriptions, some over-the-counter items prescribed by your physician and even eyeglasses. • Dependent Care Account – includes amounts you pay for daycare for children, elder care for parents or home health care for a disabled dependent.
Flexible Spending Accounts • You may contribute as little as $240 or as much as $5,000 in each type of account each year. • Health Care FSA – you may incur expenses from January 1 (or your hire date whichever is later) through March 15 of the next year and you can submit claims for that period until June 15 (of the same year). • Dependent Care FSA – you can incur expenses from January 1 (of your hire date, whichever is later) through December 31 and you can submit claims for that period until March 15 (of the next year). • You must plan your contributions carefully because if you do not use your FSA dollars you will lose them.
Flexible Spending Accounts • Regardless of which medical plan you choose, if you know you will have out-of-pocket healthcare expenses, consider the FSA. • Here’s how it works: • Your eligible out-of-pocket expenses will be $1,000 for the year, so you elect to have $1,000 withheld in equal amounts over the rest of the year from your paycheck before taxes are calculated. The full $1,000 will be available to you immediately. • As you incur eligible medical expenses, you submit a claim form and your receipt to FBMC (the administrator) and you will be reimbursed out of your account by check or direct deposit. • As you incur eligible pharmacy expenses, you can use the FSA debit card and the cost will automatically be deducted from your FSA account.
Before-Tax Premium Savings Plan • You are automatically enrolled in Metro’s Before-Tax Premium Savings Plan. This plan allows you to pay your medical, dental and vision premiums out of your paycheck before taxes are calculated and deducted from your pay. • Your taxes will be lower and your take home pay will be higher. • If you wish to opt out of this plan, please complete that section on your enrollment form.

Back-Up Child Care Program • TodayCare offers Metro employees a back-up child care option for your mildly ill children or well children whose regular child care or school is temporarily unavailable. • TodayCare is a NAEYC (National Association for the Education of Young Children) accredited center and is located at 810 Broadway, Suite 105. Hours of operation are 6:30am to 6:00pm Monday through Friday. MNPS and Hospital Authority employees are not eligible for this benefit.
Back-Up Child Care Program • $10 copay per child per visit. • Up to 30 visits per child within a calendar year, with no more than 10 consecutive days. • Reservations and walk-ins accepted. • Security procedures for drop off and pick up. • For more information, call TodayCare at (615) 250-2600 or visit www.todaycare.com.
MTA Easy Ride Program • As a Metro employee, you can now commute between home and your worksite or for any work-related trips at Metro for FREE! • By enrolling in the Easy Ride program and using your Easy Ride card, you can ride aboard MTA’s fixed-route bus or AccessRide services (with proper eligibility). • To enroll or obtain more information, contact your department’s HR representative. MNPS and Hospital Authority employees are not eligible for this benefit.
