Healthier Living : What Is the Program? • Developed by Lorig & colleagues at Stanford University in the 1990s. • Designed to help people better manage chronic health conditions and live a happier, healthier life. • Six-session once a week workshop. • Highly scripted and structured. Leaders must be trained & certified to lead this program.

Healthier Living: Why the Program is Needed • Chronic diseases affect the quality of life of 133 million Americans (nearly 1 in 2 adults)* • More than 14 million in California** • 80% of Americans 65 and older have at least one chronic condition* • 50% have at least two • Chronic diseases represent more than 80% of health care costs*** *Centers for Disease Control and Prevention, 2009 **California HealthCare Foundation, 2006 ***Robert Wood Johnson Foundation, 2004

Chronic Conditions: Impacting Quality of Life for Many Older Adults Chronic conditions account for • 76% of inpatient hospitalizations* • 88% of all prescriptions filled* • 72% of all physician visits* • 7 out of 10 deaths in the U.S. each year** *Johns Hopkins University, 2003 **Centers for Disease Control and Prevention, 2009 ***Thorpe & Walker, 2010

Healthier Living Workshop Online CDSMP (20-25 people) Consists of six weekly sessions led by 2 trained peer leaders No “set” time structure Highly interactive (discussion boards) Log on 2-3 times per week for a total of 1-2 hours • CDSMP Small Group (10-16 people) • Involves six 2½ hour sessions led by 2 trained peer leaders • Highly interactive empowerment model • Literacy level not an issue • Flexible locations customized for local needs

Workshop Objectives Managing symptoms Improving diet Increasing activity level Dealing with difficult emotions Learning relaxation techniques Developing problem solving strategies Setting weekly goals Improving communications, particularly with health providers Learning relaxation techniques Developing effective problem-solving Setting weekly goals & checking in

  8. Individual Action Plan Components Goals the participant really wants to accomplish Focuses on accomplishments that are achievable Action plan that is very specific about What? How much? When? How often? Plan includes actions they really believe they can accomplish (Confidence level of 7 or more)

Healthier Living—Outcomes for Program Completers • Greater energy/reduced fatigue • Better psychological well-being • Enhanced partnerships with physicians • Improved health status • Increased physical activity • Decreased pain, depression, shortness of breath • Decreased limitations in social and role activities • Improved quality of life • Greater self-efficacy and empowerment

Healthier Living—Outcomes for Program Completers • Fewer outpatient visits • Fewer emergency room visits • Fewer hospitalizations • Fewer days in hospital • Results in more appropriate utilization of health care resources • Health care needs addressed in outpatient settings vs. ER visits and hospitalizations • Rate of disability decline decreased

Program Target Population • Have at least one chronic condition (not disease specific) • Family members, friends or caregiver of someone with a chronic condition can also attend • Must have stamina to travel to and attend a 2 ½ hour class • Must have cognitive function to participate and implement action plan
