  1. Assessing Nurse’s Readiness for Evidence-Based Practice Shelley Thibeau, MSN, RNC Karen Rice, DNS, APRN, CNS-BC, ANP Ochsner Medical Center

  2. Background • Long history of research utilization- 1970’s • EBP models expand the sources of evidence utilized to improve practice • Clinical expertise • Expert opinion • Benchmarking data • Barriers are abundant

  3. Pravikoff, Tanner & Pierce (2005)- lack of time, lack of skills, lack of autonomy Estabrooks, et al (2003)- positive association between individual beliefs and RU Fineout-Overholt, Melnyk, & Schultz (2005)- clinical scholar programs target a core group of nursing experts Karkos & Peters (2006) Leadership strategies of Magnet™ facilities: Assessment of the readiness for evidenced based practice Development of incorporation strategies such as journal clubs to facilitate analysis of research Development of mentor programs designed to promote nursing research skills State of the Science

  4. Purpose • Strategic goal of Center for Nursing Research • Development of Nursing Scholar Program to foster integration of research into practice excellence • Needs assessment necessary for curriculum development

  5. Method • Prospective, descriptive design • Instrument - Modified survey tool (Pravikoff 2005) • 30 items • Demographics • 4-point Likert scale • Frequency and rank ordering • 2 free text responses • Protection of Human Subjects • Exempt research (OCF IRB) • Consent implied by submission of survey • Responses anonymous • Sampling Procedure • RNs recruited by flyer and email • Internet accessible for 6 weeks

  6. Demographics • Sample: 180/1200 (15% response) • Female: 96% • Mean Age: 41.7 years, SD = 10.19 • Highest nursing degree N = 180

  7. When did you receive your most recent nursing degree? N = 180

  8. Years Employed at OMC N = 180

  9. Nursing Role of Respondents N = 180

  10. When you need nursing information, how often do you? N = 180

  11. Barriers to EBP: The lack of… N = 180

  12. Need to Know Nursing Info Differences tested using Pearson’s χ2 and Fisher's exact All statistical tests were 2-tailed, and p<.05 (SAS, version 9.2)

  13. Participation in Research Differences tested using Pearson’s χ2 and Fisher's exact All statistical tests were 2-tailed, and p<.05 (SAS, version 9.2)

  14. The number one personal goal in using research to guide nursing practice… Common themes in spontaneous free text responses

  15. The most important reason for using research in nursing practice… Cumulative percentage Common themes in spontaneous free text responses

  16. Comparison to National Survey

  17. Identified needs of respondents lack of skill in Using databases Critical appraisal of research Application of research into nursing practice Desire compensated time to translate relevant research into nursing practice RNs value the importance of EBP Improving pt outcomes Enhancing RN knowledge Organizational support Compensated time to develop relevant clinical questions into EBP Continue library support, awareness, and skills Application to Practice

  18. Conclusion • Survey results Curriculum development for Clinical Investigation Internship • Compensated time for 3 interns • Series of 5 classes –build skills • Literature search and appraisal • Change management • Research • Publication • Mentored practicum tailored to project CII
