The Pain Reassessment Indicator • A new indicator to remind nurses to complete a pain reassessment after all PRN pain medication administrations • Details • When a PRN med is administered, the indicator initially displays as a clear box = • When documentation is completed correctly, the most recent “pain score” or “P” (for behavioral, and physiologic cues, etc.) will display in the indicator box • Example: Pain score • Example: P Pain reassessment is complete when documentation is entered in any one of the following HED Pain tab fields within 2 hrs of med administration: Pain Score, Response to Intervention, Post-Intervention pain score, Behavioral cues, Physiologic cues, PAINAD Score, FLACC score, , Pain intervention response (on FLACC), N-PASS Score, N-PASS Intervention Response, NIPS Score, Pain Intervention Response (on NIPS), Total Score (NWI)

2. The Pain Reassessment Indicator – Cont. • If pain reassessment is not documented within 30 minutes of the med administration, the indicator box will display a yellow background to alert the nurse that a pain reassessment is due within the next 90 minutes. • If pain reassessment is not documented within 120 minutes after med administration, the indicator will display 2 red question marks ?? to signal overdue pain reassessment. • The indicator timer will always restart with the next PRN pain medication administration. • The nurse must wait at least 10 minutes from the med administration time to document a pain reassessment, reassessments performed within 10 minutes of med administration will not trigger the indicator • Hover over the indicator for specific details • The Pain Score documentation will persist for 4 hours, then drop off the indicator display if no updated documentation is entered. PRN med list: acetaminophen w/codeine #3, alfentanil, alka-seltzer gold, aspirin, belladonna, buprenorphine, butorphanol, celecoxib, choline-magnesium trisalicylate, codeine phosphate, diclofenac, diflunisal, fenoprofen calcium, fentanyl, fiornal /codeine #3, flurbiprofen, hydrocodone/ acetaminophen, hydromorphone, ibuprofen, indomethacin, ketoprofen, ketorolac, meclofenamate, meloxicam, meperidine, methadone, methyl salicylate / menthol cream, morphine, nabumetone, nalbuphine, naproxen, oxycodone, oxymorphone, pentazocine, prppoxyphene, remifentanil, salsalate, sufentanil, tramadol, trolamine salicylate
