  1. Recurrent Stroke and its related factors among hypertension patients in Thailand. Present By: Nitchamon Rakkapao Dr. PH student

  2. Background: • Recurrent stroke is an important health problem in worldwide with huge burden a human health. • It is common complication of hypertension patients was thought high rate of mortality and disability. • A few data of recurrent stroke in developing countries has been reported including Thailand.

  3. Research question • What is the magnitude of recurrent stroke among hypertension patients in Thailand. • What are the factors association with recurrent stroke among hypertension patients in Thailand.

  4. Objectives: • Primary objective To determine the prevalence of recurrent stroke among hypertension patients in Thailand. • Secondary objective To study factors associated with recurrent stroke .

  5. Primary outcome • Prevalence of recurrent stroke among hypertension in Thailand. • (Stroke included ; cerebrovascular accident ,ischemic, hemorrhagic, stroke of unknown type and Transient Ischemic Attack (TIA) • Outcome measurement: Having stroke 2 time was diagnosed by Doctor.

  6. Methods: • This study is a part of DMHT database in Thailand, 2010-2012 • As Cross-sectional study. • Data was collected from medical records by health officer. • 104,235 cases of HT • Multiple logistic regressions were used for the data analysis.

  7. Total survey n=3,373,089 Consort diagram DMHT in 3 years (n =174,578) Excluded ;2010 year (n=53,119) DMHT in 2 years (n =121,459) Excluded ;DM (n=17,224) HT (n =104,235) Excluded ;no stroke (n=100,796) Stroke patients (n =3,439)

  9. Results

  10. Prevalence of recurrent stroke Prevalence rate 1.5% (95% CI;1.15-2.01)

  11. Prevalence of recurrent stroke

  12. Prevalence of recurrent stroke

  13. Prevalence of recurrent stroke

  14. Results (cont.)

  15. Results (cont.) Multivariate logistic regressions analysis for recurrent stroke * Adjusted for baseline measurements and other covariates including ,HDL-cholesterol, diastolic bp, type of cerebrovascular and sex.

  16. DISCUSSIONS: • Our prevalence was lower than previous reportsfrom Europe and some countries of Asia • (In United States was 3-40%, Spain was17.6% , Scotland was10.8,% , Australia 19.8%was Japanese was 9.6% ,and China was 11.2%,respectively)

  17. DISCUSSIONS: (cont.) • However, the rate of recurrent stroke was only 1.5 %, stroke patients should continually to focus on the medications and self-care. • This study may underestimate the problems, for examples, some patients with recurrent stroke may die before data was collected.

  18. DISCUSSIONS: (cont.) • Regarding in this study, every 1 mmHg of diastolic blood pressure increased, HDL-cholesterol less than 40 mg/dL and having hemorrhagic stroke and being a female related with recurrent stroke • For Blood pressure, hemorrhagic stroke and sex This finding were consistent with previous many study. • For the effect of HDL-cholesterol levels should be evaluated in preventing recurrent stroke in the future studies.

  19. DISCUSSIONS: (cont.) • A limitations of this study is that some variables were not explored such as duration before occurred recurrence, lifestyle and self-care behaviors

  20. Conclusions:(cont.) • Prevalence of recurrent stroke in Thailand was lower than that of previous reports in Europe and in some countries of Asia. • The preventive strategies still need to proceed in the future, to improve quality of the patient's stroke. • The effect of HDL-cholesterol levels should be evaluated in preventing recurrent stroke in the future studies.
