Major Excursion Consent and Medical Information Form – Off Campus Excursions • Can be done as a ‘once a year’ form at the beginning of the year. • Must be done for all Swimming and Water Safety programs and any activity involving swimming or the possibility of submersion (sailing, canoeing, rafting, etc.) • Date of first activity and last activity are included • Must attach a separate itinerary/calendar of events/activities that parents can keep. Dates can be approximate.
Major Excursion Consent and Medical Information Form – Off Campus Excursions • Must advertise/remind parents of the upcoming activities in the week(s) prior to the actual date • Must ask parents to advise the school of any changes to personal information/emergency contacts • It’s about parents making ‘informed’ consent • Risk Management Plans are an integral part of the process and must be signed off by Principals

Consent Form for Aquatic (Water-based) Activities • Must be done for all Swimming and Water Safety programs and any activity involving swimming or the possibility of submersion (sailing, canoeing, rafting etc) • Can be done with the Major Excursion Consent and Medical Information form, once a year. • Must have all activities listed with dateand venue. Some schools have a reference to using alternative venues if a change becomes necessary.

I understand that water-based activities/swimming will be occurring and that staff with appropriate first aid, CPR and rescue qualifications will be present. In the event of accident or illness, when it is impracticable or impossible to communicate with me, I understand that the teachers or their delegates at the venue will arrange for my child/ward to receive such medical or surgical treatment as may be deemed necessary, and/or administer such first aid as may be judged necessary. These will be the aquatic activities your child/ward may participate in as part of the School Outdoor Education Program: I ____________________________ (Parent/ Guardian) give permission for my child/ward to participate in the activities included above as outlined in the Outdoor Education Subject Handbook. I understand that water-based activities/swimming are part of the Outdoor Education program and that staff with appropriate first aid, CPR and rescue qualifications will be present. SARIS – SWSP Results SARIS Data entry – • Primary and District High Schools Only • Became a requirement after Coroners report • Only 5 results from 16 need to be entered • Days Attended (DAT), Boat Safety (BSA), At Risk (ATR), Continuous Swim (CSW), 3 Minute Survival Swim (SSW) • School based decision as to who enters data
SWSP Framework in SARIS 1. Click on the column header to open a framework for a whole group of students or click on the grid to open a framework for an individual student.
ENTERING ASSESSMENT RATINGS FOR A WHOLE CLASS 1. The easiest method is to use the down arrow view. Click on the arrow at the top of the field that you wish to use. In this example we are going to enter Swim and Water Safety Program ratings.
