New Mishap 1 First Aid only and Matter of Record can be closes by Wg/CC
New Mishap 1 Do not give too much information. Just basically say what happened. Do not enter names , tail numbers or locations. Good example: SM twisted his right ankle climbing out of a CAP van.
Update New Mishap 2 (Initial notification) Initial notification data already entered
Update New Mishap 2 (Additional Information) • Purpose of activity • CAP activity, mission number, photos taken • Weather • Follow-up narrative
Update New Mishap 2 (Add Person to Mishap) • Add people who know facts of the mishap • Each member listed will receive an email asking for their factual input
Update New Mishap 2 (Submit) • You must populate each tab before submitting • Click the “Submit” button at the bottom of the page to complete
Manage the Mishap Report Select the report that you want to manage
Manage the Mishap Report Initial account of the incident can be edited. (Remove personal information) Wing Commander can select a Reviewing Office People involved can be added or deleted Files and photographs can be added and reviewed
Manage the Mishap Report(lower part of the page) This is where the Reviewing Officer puts his final report Select this botton to send the completed report Wing Commander for his review
