WellOne is…… • A 501 (c)(3) non-profit agency • Governed by a volunteer board of directors with a 51% consumer majority • Licensed by the State of Rhode Island • An FQHC (Federally Qualified Health Center)
What is a FQHC? • Federally qualified health centers (FQHCs) include all organizations receiving grants under Section 330 of the Public Health Service Act (PHS). FQHCs qualify for enhanced reimbursement from Medicare and Medicaid, as well as other benefits. • FQHCs must serve an underserved area or population, offer a sliding fee scale, provide comprehensive services, have an ongoing quality assurance program, and have a governing board of directors.
Our Mission • To provide excellent primary medical and dental care and behavioral health services to the residents of the Rhode Island communities that we serve. • Our emphasis is to promote the highest quality of health for the individual and family as well as for the community.

Our Vision • WellOne is the primary medical and dental care and behavioral health provider of choice for the residents of the Rhode Island communities that we serve. As a result of our efforts, these residents have achieved the highest attainable level of physical, psychological and social well being.
History • WellOne is the uniting of two great non-profit community health centers, Northwest Community Health Care and Bayside Family Health Care, with a combined history of serving the needs of Rhode Islanders for over 130 years. • From its inception, WellOne has demonstrated the ingenuity of local efforts and how a community can come together to provide healing, relief, and comfort to our citizens and respond to their evolving needs. • Originally established in 1909, WellOne has grown from a single nurse in a horse and buggy caring for one village to 100 employees in three locations serving patients from throughout Rhode Island and nearby Connecticut and Massachusetts
Over the last 100+ years… Pioneer in public health: • First Mumps clinic in the nation • First Measles clinic in the state • First Rural Health Clinic in nation • Groundbreaking immunization and chronic disease management programs • Dental clinics dating back to 1920s • Innovative primary medical-behavioral health collaborative care
Facts • Serves 13,000+ patients annually • Majority have private insurance (46%) but 15% are uninsured and 25% have Medicaid and 15% Medicare • 73% have income levels at or below 200% of the federal poverty level ( individual: ~$22,000 or less; family of four: ~$46,100 or less) • In 2012, provided $938,000 in charity care or 9% of our current budget
In 2012….. • WellOne completed 29,917 medical encounters, 4,627behavioral health encounters and 16,312 dental encounters • 83% of income was generated from direct patient care services, 10% from federal funding and 7% from other sources • Commitment extends beyond the four walls of health centers: advising struggling patients of eligible assistance program; guiding families with at-risk children in need of comprehensive socialservices; providing school-based oral screenings and consultative medical services; offering nursing-home based dental services for Medicaid patients; conducting free blood pressure screenings at local community centers; partnering with the West Bay YMCA to prescribe and support healthy remedies for patients; and hosting or participating in community events

PRIMARY CARE • WellOne provides a full scope of primary care services including well care, health maintenance, treatment of chronic and acute disease processes • Our providers are board certified in their specialty area. • We treat patients of all ages, from birth to geriatrics • Convenient hours are available with same day appointments for those with urgent needs.
Dental Care • We provide a broad range of preventative and restorative dentistry • Our staff consists Dentists, Dental Hygienists and Dental Assistants
Behavioral Health • Behavioral health services are offered at One High Street in Pascoag and at the North Kingstown office. • Services are available for children, adolescents, adults and families.
Pharmacy services • Under the 340B program, uninsured and underinsured patients of WellOne are eligible to realize significant savings on pharmaceuticals • CVS is our community partner for providing these pharmacy services to our patients
On site Laboratory • On site laboratory services are provided by Lifespan (Pascoag and Foster locations) and Care New England (North Kingstown location) • These laboratories accepts most insurances and financial assistance is available for patients in need • Convenient hours are available, including Saturday hours at most sites; no appointment is necessary
Special Programs • Family Resource Counseling (FRC): FRC services are available to provide comprehensive eligibility assistance as well as coordinating care and procuring resources for patients. • Title X: At the Pascoag site, WellOne provides a broad range of family planning services under this program, including counseling, provision of contraceptive methods, pregnancy testing and STD screening, including rapid HIV testing and counseling. • Nursing home care: WellOne provides primary care and dental services to our partner long term care agencies. • School Services: WellOne is designated as the school physician and dentist for the Burrillville School System and as the physician for the Smithfield School System. (Services provided include but are not limited to: sports physicals and dental screenings.)
Special Programs cont. • All community health centers, including WellOne, participate in the Rhode Island Chronic Care Collaborative (RICCC), an in-state collaborative effort to examine innovative ways to manage patients with chronic illnesses. • Diseases focused on are diabetes, asthma and colorectal cancer.
Community Involvement • WellOne recognizes the essential responsibility of maintaining close community partnerships. • WellOne participates in community health fairs, local wellness groups, community task forces, and community based blood pressure screenings.
Funding sources • Operations are funded through patient/insurance contributions, Federal 330 funding, and various grants/charitable contributions.
Payment sources • WellOne accepts most major health care insurances. • Sliding fee services (rates based on ability to pay) are available for patients who meet federally designated income criteria.
