  2. KEY PROBLEM • With the implementation of routine antenatal administration for all Rh neg women, we had an incident of 1 client not having Anti-D • Clients attending the ANC were seen by different clinicians at each visit, the potential to miss giving Anti-D was seen as an on-going risk

  3. AIM OF THIS PROJECT • Simple prompt – a brightly colored label –to remind all clinicians to give Anti-D at the appropriate visit • All areas offering ANC to be involved and educated • Once developed –to be put in place ASAP • Limited expense, using what resources we had

  4. KEY CHANGES IMPLEMENTED • Sticker developed and placed in the front cover of medical record • All staff caring for ante-natal clients informed of the changes • Changes made to the guideline relating to the administration of Anti-D

  6. OUTCOMES SO FAR • The label was implemented in January 2009 • 46 labels have been placed in medical records • To date no anti-d has been missed

  7. PROJECT EVALUATION • This was a simple project, easily implemented, and to date effectively completed • Review in 12 months to assess effectiveness of label and staff satisfaction with label
