10. FATALITY WITH BUCKET TRUCK An employee was using a bucket truck and had been given the task of painting a metal utility pole with a spray gun. For some reason, he leaned back in the bucket and the back of his neck came in contact with a 7960 volt bare wire. The employee was knocked out of the bucket and into the bed of the personnel hoist truck

11. FATALITY WITH BUCKET TRUCK Two employees fell out of a "cherry picker" when the basket they were in was struck by a passing tractor trailer. Both employees were wearing personal fall arrest equipment but were not tied off to the basket. One employee was fatally injured when he fell 13 feet to the concrete surface.

12. FATALITY WITH BUCKET TRUCK An employee was riding on the left side of a bucket truck and fell off. He was run over by both left wheels and died. An ironworker struck by overhead beam while booming basket lift backwards resulting in asphyxia and death.

Survey the area around equipment for uneven ground surface. Check for oil or hydraulic fluid leaks. Look for people behind, in front, or under the machine
