Introduction- Culture in health care • US society is increasingly culturally diverse • Healthcare industry is culturally diverse • 44% ACGME resident positions FMG’s • 40+% of our residents and medical students are foreign born • Do cultural differences influence a healthcare educational environment?
What is Culture? • IBM – Hofstede international studies • Proposed elements of culture • Power Distance Index : Social Stratification • Individualism vs.. Collectivism • Tolerance of Uncertainty • Masculine-Feminine • Long term Outlook • Quantifiable in populations
Power-Distance Index : Social Stratification
Masculine – Feminine Tolerance of Assertiveness
Uncertainty Avoidance Risk-Taking
Objective • The aim of this study was to measure cultural characteristics or dimensions amongst faculty, residents and medical students at an urban medical school as it may relate to the quality and effectiveness of health care delivery.
Materials & Methods • Hofstede’s Value Systems Module (VSM08) questionnaire with 35 multiple choice questions were administered to faculty, residents and students. • Calculated cultural dimensions including Power Distance Index (PDI), Individualism (IDV), Masculinity (MAS) , Uncertainty Avoidance Index (UAI) and Long- term Outlook (LTO) were compared amongst these groups. • Statistical correlations were measured using SPSS
Implications for Education • Is there a definable “Healthcare Educational Culture?” Is it “American?” • Are faculty and students in agreement/alignment? • Is the educational “culture” influenced by non-American cultures? • Is cultural misalignment a problem?
Conclusion • There are significant differences in the cultural dimensions between faculty and students. • These are influenced by age, gender and immigration. • Understanding these differences would ensure appropriate evaluation of educational outcomes in medical students.
