Developing our System Operating Plan 2019-20 Key Messages Pack December 2018
Introduction • Local Authorities and local health organisations are working together as the Frimley Health and Care System to provide a joined up health, care and wellbeing system • Frimley Health and Care will produce a single system Operational Plan for 2019-20 reflecting the development of our Integrated Care System, partnership working and including our Organisational and Local Place-based Priorities • We are sharing our planning process with Frimley Health and Care organisations, staff, stakeholders and local communities through monthly updates. For November’s briefing pack please right click here. • We will be inviting opportunities for discussion via email and encouraging feedback from you and how you can play a part • During November we have: • Held a joint workshop with ICS partners to identify how we can work together to get best use from the Frimley £ and bring the system to financial sustainability • Produced an early draft of the 2019-20 Frimley Health and Care Operational Plan to enable feedback • How you can help: • We would appreciate your feedback/comments on the first draft Plan narrative to help inform the development of the second draft of this Plan • Please would you send any comments/feedback to helen.single@nhs.net (copying in Diane Sheffield – diane.sheffield1@nhs.net) no later than Friday 14th December • There will a be a further opportunity to comment in late January/ early February when the 2nd draft Plan will be produced 2
Frimley Health and Care are required by NHSE/I to produce a refreshed one-year operational plan for 2019-20 We will refresh and build upon the 2018-19 ICS Operational Plan with any adjustments made once national planning guidance is published Set within a 12 month planning improvement journey which will inform and support delivery of multi-year plans - within the context of the longer term national 10 year plan We want One Plan that includes system, organisationaland local place-based priorities Currently first draft of the One Plan has been produced – refresh of ICS programmes and enablers/ some new chapters included Operational Plan Narrative 2019-20 3
Strategic Approach All of the partners in the Frimley Health and Care ICS are committed to putting residents first. In practice this translates to people receiving/having access to seamless holistic care that meets their need at the earliest possible opportunity. Through focussing on the individual, as opposed to the structure, we want to deliver an increased focus on prevention and pro-active care rather than reactive treatment. The partners are taking collective responsibility for simplifying the system and making it easier for people to understand and navigate it. Over time we want to see people leading healthier lives for longer across our population and local neighbourhoods. To achieve this we will: Be a system with a collective focus on the whole population we support throughout their lives, not a system based on sectors, organisations services or parts of the population Work in increasingly integrated ways across disciplines and across organisations for the greater benefit of our local communities Move our resources over time to those places of most need of investment to improve care and well-being and maintain the stability of our component organisations. This will include a shift in investment over time to our out of hospital provision, a greater proportion of each annual allocation moving towards general practice, community and mental health provision and our prevention and self care agenda so that we support people to remain as healthy, active and independent as they can be Achieve greater equity across health and care in our geography, so that those with the greatest need are a focus of priority investment and interventions, rather than all receiving the same irrespective of need Use our population health management tools and intelligence to target improvements more and more accurately to those points of greatest need and delivering the greatest value Address the wider determinants of health for those residents most vulnerable to environmental factors Deploy digital advances and automated tools to support individuals and professionals to make better choices with more information, increasing the ability and confidence of our local communities to take responsibility for their own health, care and wellbeing Invest in developing the right workforce with the right skills, knowledge and understanding to transform our services and pathways System Priorities 4

GP Transformation: Lay foundations for a new model of general practice provided at scale, including development of GP networks to improve resilience and capacity Children and Young People Priority 2: Action to improve long term condition outcomes including greater self management and proactive management across all providers for people with single long term conditions Cancer 4. Support Workforce: Design a support workforce that is fit for purpose across the system Enablers Local Priority 3: Frailty Management: Proactive management of frail patients with multiple complex physical and mental health long term conditions, reducing crises and prolonged hospital stays 5. Care and Support: Transform the social care support market including a comprehensive capacity and demand analysis and market management Workforce Analytics . Priority 4: Redesigning urgent and emergency care, including integrated working and primary care models providing timely care in the most appropriate place 6. Reducing clinical variation: Reduce clinical variation to improve outcomes and maximise value for individuals across the population Enablers Estates Priority 5: Reducing variation and health inequalities across pathways to improve outcomes and maximise value for citizens across the population, supported by evidence Digital & Technology 7. Shared Care record: Implement a shared care record that is accessible to professionals across the ICS footprint Communication & Engagement
