Missing Homework Deficiency Slip Name_______________________ Date________________ Dear Parents: Today your child did not have in his/her possession a homework assignment that was due. Although this assignment must still be made up, a deficiency slip serves as a communication between the parents and teachers, so as to reinforce the student’s accountability for all assignments given. Three deficiency slips in one subject, may result in the lowering of your child’s overall performance on assessments in that subject area and result in a parent conference. Please discuss with your child the importance of having all completed homework assignments in school. Your cooperation and concern with this matter is greatly appreciated. Subject: Sincerely, Assignment: Mrs./Mr Parent’s signature______________________________
