CCG Care Home Service Improvement ProjectsInvolvement with – ENRICH (enabling research in care homes) , NHS Clinical Skills and Dementia Care MattersThe social care telecare pilot has now come to an end, however care homes can purchase or lease equipment by contacting Heather.Barden@york.gov.ukAdvanced Care Planning Training25/78 homes in the area provide nursing (dual registered )care and are supported by the community matron serviceResidential homes can access support through the general district nursing service, and other NHS support is also available through the medicines management team and NHS care homes psychiatric liaison teams.
