Consistent with national trends, the State and City of New York have achieved unprecedented success over the past several years in moving individuals from welfare to work.
In 1995, 1,160,593 individuals were receiving public assistance in New York City. In April, there were 438,062 PA recipients, a decline of 62%.
4. CLINICAL BARRIERS TO EMPLOYMENT A significant number of people remaining on welfare have complicated clinical barriers to employability including medical, mental health, and substance abuse conditions.
HRA has created a division, Customized Assistance Services, to develop programs that provide comprehensive individualized and clinically focused services for these hard-to-serve-groups.
Building on our experience, HRA developed a new initiative to expand services and improve outcomes for individuals with health/mental health barriers to employment.

WeCARE- Wellness, Comprehensive Assessment, Rehabilitation, and Employment.
6. WeCARE is HRA’s new initiative that will assess PA clients with potential health barriers and provide a range of services to help them achieve wellness and self-sufficiency.
WeCARE will service over 45,000 PA applicants and recipients annually.
WeCARE will work with two primary contractors, and their subcontractors, to provide services.
8. Based on the functional outcome, WeCARE will: Develop a comprehensive service plan to address the individual’s health, mental health, social barriers to employment including those that result from problems in their family.
Provide case management services as needed to facilitate wellness, rehabilitation and self-sufficiency.
13. WeCARE WeCARE incorporates the lessons learned from our current practice in working with individuals with health/mental health conditions. New and important elements are:
Holistic assessments
Continuum of integrated services from assessment to rehabilitation to self-sufficiency under one umbrella contract
Clinical focus and support
Proactive Wellness Plans to facilitate compliance with treatment
Vocational rehabilitation starts with a diagnostic vocational evaluation
Case management services
