  5. Participant Flags in APPS Critical Findings – Cannot be cleared by local HUD Office 1. Unacceptable Physical Inspection Score (<60 pts) 2. Below Average or Unsatisfactory Management Review 3. Mortgage Assignment or Foreclosure 4. Default under HAP Contract or violation/breach of HAP 5. GSA Excluded Party Listing Name Match 6. Regulatory Agreement Violation or Breach 7. Debarment, suspension or temporary Denial to Participate 8. “NO” answer on a Previous Participation Certification 9. Failure to file financial statements
