Insulin Resistance and Syndrome X • Abdominal obesity is the form of obesity most strongly associatedwith the metabolic syndrome. It presents clinically as increasedwaist circumference. • Atherogenic dyslipidemia manifests inroutine lipoprotein analysisby raised triglycerides and lowconcentrations of HDL cholesterol.A more detailed analysisusually reveals other lipoprotein abnormalities,eg, increasedremnant lipoproteins, elevated apolipoproteinB, small LDL particles,and small HDL particles. All of theseabnormalities have beenimplicated as being independently atherogenic. • Elevated bloodpressure strongly associates with obesity andcommonly occursin insulin-resistant persons. Hypertension thuscommonly islisted among metabolic risk factors. However, someinvestigatorsbelieve that hypertension is less "metabolic"than other metabolic-syndromecomponents. Certainly, hypertensionis multifactorial in origin.For example, increasing arterialstiffness contributes significantlyto systolic hypertensionin the elderly. Even so, most conferenceparticipants favoredinclusion of elevated blood pressure asone component of themetabolic syndrome. • Insulin resistanceis present in the majority of people withthe metabolic syndrome.It strongly associates with other metabolicrisk factors andcorrelates univariately with CVD risk. Theseassociations, combinedwith belief in its priority, accountfor the term insulin resistancesyndrome. Even so, mechanismsunderlying the link to CVD riskfactors are uncertain, hencethe ATP III’s classificationof insulin resistance asan emerging risk factor. Patients withlongstanding insulinresistance frequently manifest glucoseintolerance, anotheremerging risk factor. When glucose intoleranceevolves intodiabetes-level hyperglycemia, elevated glucoseconstitutes amajor, independent risk factor for CVD. • A proinflammatorystate, recognized clinically by elevationsof C-reactive protein(CRP), is commonly present in personswith metabolic syndrome.Multiple mechanisms seemingly underlieelevations of CRP. Onecause is obesity, because excess adiposetissue releases inflammatorycytokines that may elicit higherCRP levels. • A prothromboticstate, characterized by increased plasma plasminogenactivatorinhibitor (PAI)-1 and fibrinogen, also associateswith the metabolicsyndrome. Fibrinogen, an acute-phase reactantlike CRP, risesin response to a high-cytokine state. Thus,prothrombotic andproinflammatory states may be metabolicallyinterconnected.

Tests • Glucose (alone or part of metabolic panel) want in 80’s or below. • Fasting Insulin – ideal at 2 or below • HgA1C – below 6 • Lipid Panel • Triglyceride/HDL ratio (2 or less is considered ideal4 – high, 6 - much too high) • C-Reactive Protein (systemic inflammation) • Ferritin - important independent risk factor for heart disease • Waist circumference • women 31 inches = slight health risk, over 35 inches = substantially increased health risk. • Men - Waist of over 37 inches = slight health risk. Over 40 inches = substantially increased health risk.
Sugar in the blood stimulates secretion of insulin from the pancreas. Insulin binds to receptors on the membrane, triggering intracellular communication.
These vesicles fuse to the membrane surface and provide transport for sugar into the cells to be utilized for the production of energy by the mitochondria. The insulin signal on the membrane’s surface stimulates production and mobilization of vesicles containing specialized GLUT 4 molecules, responsible for membrane uptake of sugar.
Stimuli like insulin and exercise cause it to move to, and become active by embedding in, the cell membrane. Impairment of insulin stimulated movement of GLUT-4 is the most obvious cause of insulin resistance. GLUT-4 is the main insulin sensitive transport protein embedded in the cell membrane and is responsible for transporting glucose into the cell.
Receptors on the nuclear membrane (PPAR Alpha and Gamma), activated by certain fatty acids, regulate gene expression that contributes to proper glucose homeostasis and energy production in the mitochondria.
Conjugated linoleic acid (CLA) binds to PPAR Gamma and Alpha, stimulating nuclear transcription. The agents produced as a result are key to proper glucose homeostasis and energy production.
An unhealthy diet, high in refined carbohydrates, causes rapid elevation of blood sugar. The pancreas responds by increasing insulin output.
If the capacity of the insulin receptors is compromised due to nutrient insufficiencies, the internal message to the cells is reduced and the efficiency of the cell is compromised.
The insulin signal can be further attenuated by inflammatory mediators such as NF-KappaB and cytokines such as TNF alpha. When this happens, fewer GLUT 4 vesicles are formed and glucose uptake is compromised. X
Gene activation by PPAR Alpha and Gamma may also be compromised, leading to further reduction in the cell’s ability to produce energy.
