Required Paperwork • Permission Slip (required) • Medical Information Form (required) • Parent/Guardian Consent and Release Form (required) • Medication Form • Only necessary if medications will be needed • Prescription • Over-the-counter • Must be completed by the doctor • All forms are due between September 15-19
Trip Financing • Total Cost of the Trip: $255 • Fundraiser: Joe Corbi’s • Packets distributed this week • Orders must be submitted between August 26 – September 5 • Each item sold will earn the student $5 toward defraying the cost of their trip • Pick-up date: October 1st from 3-6 pm • September 11: Balance sheets will be distributed • Balance payments must be submitted Sept. 15-19
Chaperones • 15-20 chaperones are needed at any time during the trip/school • If interested, complete the chaperone form and send it in to your child’s Period 1 teacher (Bonnie Branch – by September 5th), or to Tim Pruett (Mayfield) • Chaperones will be notified and confirmed before September 15th. • No charge for chaperones
Chaperones • Chaperones have three choices of coverage: • Stay the entire trip (10/20-10/24) - preferred • Stay the first half: (10/21-10/23 at 2 pm) • Stay the second half (10/23 at 11am-10/25) • Please understand that, for the sake of the program’s consistency, if there is an abundance of chaperone applications those able to stay the entire trip will be given priority.

Environmental Program • In line with the voluntary state curriculum • Hands-on • Emphasis on research • Trans-disciplinary: Integrates math, reading, language arts and social studies objectives
Adventure Activities • Zip Lines • Indoor/Outdoor Rock Climbing Walls • Ropes Course • Kayaks • Giant Swing • Perceived risk vs. actual risk
Dining Hall • Food Served Family-style • Students responsible for setting up and cleaning up after meals • Special dietary needs must be documented on the Health Form and the staff will do their best to meet these needs.
Free Time Activities • Socialize in the lodge • Organized activities in the gymnasium • Organized outdoor activities • Frisbee golf course • Snack bar/gift shop
North Bay Cabins • Each cabin holds 8-9 students and 1-2 parents • Cabins have full bathrooms • Air conditioned/heated • Linens provided
