    2. Introduction PROCEDURES FOUND IN FAA ORDER 8130.21A The Form is consistent with the Joint Aviation Authorities (JAA). Is basically a MULTI-USE-use form.

    3. BACKGROUND THE FORM ORIGINALLY ADDRESSED ISSUES REGARDING EXPORT OF AVIATION PRODUCTS AND PARTS. WAS ORIGINALLY DESIGNED TO SATISFY THE ACCEPTANCE OF FAA APPROVED PARTS BY OTHER COUNTRIES.

    4. THE EVOLUTION OF FORM 8130-3 The FAA determined that the form could be used to aid in its effort to combat against the installation of UNAPPROVED parts. The tag was revised in January of 1994 to permit additional functions.

    5. WHAT CAN THE NEW FORM DO? 1.) It can be used to document conformity inspections for Class II and Class III products. 2.) It can be used to identify “new” parts and products that are being shipped.

    6. FORM USE (continued:) 3.) It can be used as an export AIRWORTHINESS approval for Class II and Class III products. 4.) It can be serve as a return to service document after maintenance, preventive maintenance and alteration.

    7. FORM 8130-3 for CONFORMITY INSPECTIONS Used by the manufacturer for conformity determinations. Only at the request of the importing country’s CAA. Only for new or newly overhauled products. May require additional paperwork.

    8. FORM 8130-3 FOR IDENTIFICATION OF NEW PRODUCTS Used by manufacturers to identify parts and assure traceability. Provided technicians with proof that part is FAA Approved. Manufacturers using the form for identification purposes, must first have an established procedure in their Quality Manual.

    9. FORM 8130-3 FOR EXPORT APPROVAL May be used for Class II products (FAR 21.323) by any exporter or its representative... ...in other words, any manufacturer, distributor, or private citizen...as long as it is signed by an FAA Inspector, or an an appropriately authorized FAA designee (DMIR or DAR). May be used for Class III products ONLY by a manufacturer or who has in their emply a designated represenitive of the administrator

    10. Form 8130-3 FOR EXPORT APPROVAL FOR USED PRODUCTS Signed by a certificated U.S. air carrier or repair station for return to service. May be used in lieu of the return to service signatures of a U.S. air carrier or repair station...providing the importing CAA confirms in writing that the part will be excepted as being a used part and that such an export approval is acceptable to that country’s CAA.

    11. FORM 8130-3 FOR RETURN TO SERVICE For Class II and Class III products...as long as the work was performed by a Part 135, 121 air carrier or Part 145 repair station. The Form must contain all information required by FAR 43.9 maintenance record entries. Procedures for use must also be included in the air carrier’s GMM or the repair station’s IPM manuals.

    12. RETURN TO SERVICE (continued:) Major repairs or alterations must have either a FAA Form 337, or a signed copy of the repair station work order as required by Appendix B of Part 43 Or... a copy of the air carriers records indicating a major repair or alteration has been accomplished. Any attachments MUST be referenced on the 8130-3 form.

    13. RULES OF THUMB FOR THE 8130-3 FORM MAKE FORM OUT IN DUPLICATE ONE COPY WITH THE PART ONE COPY AT THE REPAIR FACILITY ONLY USE THE 8130-3 FOR ONE FUNCTION AT A TIME....EXAMPLE: IF USED FOR PART IDENTIFICATION...DO NOT REUSE THE FORM FOR RETURN TO SERVICE...GENERATE A NEW FORM.

    14. OTHER THINGS TO REMEMBER THERE MUST BE AN APPROVED METHOD OF TRACKING USED IN BLOCK 3 OF THE FORM. THIS SYSTEM MUST BE EXPLAINED IN THE QUALITY MANUAL OF THE MANUFACTURER, THE AIR CARRIER’S GMM, OR THE REPAIR STATION’S IPM. THE FORM MAY BE COMPUTER GENERATED...BUT NOT CHANGED IN ANY WAY.

    15. ADDITIONALLY: If used for conformity determination or AIRWORTHINESS approval, the form a separate form may be used for each part, or multiple part numbers/serial numbers may be used. multiple items should be numbered in sequence. If a list is generated...Block 6 should say: “See List Attached” If used as a return to service document...the “Remarks” section must contain all the required data required by FAR 43.9
