Mission Statement To support the development of skills needed to be self-sufficient in the community
Eligibility for Young Adult Services Johnson County Resident Between the ages of 18 and 24 years old Meets criteria for severe and persistent mental illness (SPMI), or severely and emotionally disturbed (SED) Exhibits need for level of service to be successful in the community (Johnson County Mental Health 2000).
Services Provided through Young Adult Transition Program • Case Management • Skills Teaching • Medication and symptom education • Psychosocial Rehabilitation Group/Individual • Targeted Case Management • Supported Employment • Supported Education • Resource Acquisition • Crisis Intervention
Collaboration within the community • Young Adult Transition Program’s Mission supports the goal to connect transition aged youth with natural occurring resources and more permanent supports. We utilize community resources for the following: • Independent living • Socialization • Employment • Education • Housing • Family and individual education regarding mental illness and support groups
Diagnoses that manifest in individuals with ASD or differential diagnosis: Mental Retardation Psychosis Obsessive-Compulsive Disorder – Social Phobia and other Anxiety disorders Attention Deficit/Hyperactivity Disorder Depression (American Psychiatric Association [DSM-IV-TR], 2000)
Challenges facing providers and consumers with ASD in the community mental health system How to integrate multi systems addressing individual needs that adopt differing treatment philosophies and treatment approaches to reach the same successful goal.
