Past successes • 1st PBC group to achieve level III • Lead on COPD pathway • Lead on Diabetes Care • Enhanced Primary Mental Health Service (1st wave) • Prescribing incentive scheme and reduced costs • Ophthalmology – new service • Counselling-kept it within primary care • Physiotherapy- equity in access across the patch

  4. Failures • Failed to appoint PCT manager • Failed to join in CATS and CAS services • Failed to get PARS • Navigant asked to help

  5. Working with the PCT • Commissioning of the UCC at Hemel Hempstead • Commissioning of the OOH • Reconfiguration of the intermediate care team and having intermediate care beds • Rapid Assessment unit • The “New Hospital” at Hemel

  6. The Future • Restructuring following Navigant report • Referral management scheme (could one call it a CAT?) • Looking at physiotherapy service • Introducing scriptswitch • Working with the PCT to look at a Rapid Assessment Unit at the UCC and providing 8am till 10pm intermediate team 7 days a week • The new Dacorum “Hospital” • Continue to work with PCT on other projects
