The Problem • Ensuring continuity of care between • Bridging gaps between shifts and care settings • Providers, institutions and professions. • For patient safety issues • There are many summaries and transfer forms • Nursing data does not always appear on the form • Nursing data does not electronically move from the point of care origin to the next shift, care setting or institution

  3. Use Case • The Home Care Nurse (HCN) admits a patient, Ann, newly diagnosed with Diabetes Mellitus (DM) during a hospital admission for a Myocardial Infarction (MI) • Ann is has increased anxiety related to her fear of sudden death due to potential of subsequent MI. • HCN finds it difficult to teach Ann and provide the education needed for Ann to heal and manage her disease processes.
