Objectives • Understand the reasons to refer to an SLP • Identify the SLP assessment methods for dysphagia and communication impairments • Identify types of AAC candidates and devices
Speech Language Pathology Refer to an SLP for the following concerns: • Swallowing or eating difficulties • Difficulty communicating • Difficulty being understood • Word finding problems • Memory and cognition deficits • Voice Changes
Speech Language Pathology Evaluation and diagnosis • Dysphagia • Communication Impairments Following the evaluation, recommendations and an individual treatment plan is developed
Dysphagia in Neurodegenerative Disease • As many as 57% of patients with NDD will be impacted by Dysphagia. • Variable presentations by diagnosis • Early detection helps prevent dehydration, malnutrition, weight loss, and aspiration pneumonia • (Walshe, et al., 2014).
Dysphagia • What can an SLP do for dysphagia? • Assess the patient’s risk for aspiration and possible dysphagia • Clinical Evaluation of Swallowing • Modified Barium Swallow Studies • FEES • Recommend strategies of diet modification or meal time strategies for safe swallowing • Implement therapy • Compensatory or rehabilitative • Exercise • Specialized therapy techniques • Neuromuscular Electrical Stimulation (NMES)
Communication deficitsin Neurodegenerative Disease May present as: • Motor Speech Disorders • Apraxia • Dysarthria • Aphasia • Cognitive-linguistic deficits • Voice Disorders • Any combination of the above
Assessment of Communication • General assessment categories • Apraxia • VisiPitch Motor Speech Profile • Informal speech tasks • Informal and audiotory - perceptual judgement • Dysarthria • Assessment of Intelligibility of Dysarthric Speech • Frenchay • VisiPitch Motor Speech Profiles • Informal and audiotory - perceptual judgement • Aphasia • Boston Diagnostic Aphasia Examination • Boston Assessment of Severe Aphasia • Western Aphasia Battery • Boston Naming Test • Cognitive-linguistic deficits • Assessment Battery of Communication for Dementia • Cognitive Linguistic Quick Test • Ross Information Processing Test • Voice Disorders • VisiPitch • Sound Level Meter
Speech Therapy for Communication • Develop an individual treatment plan • Rehabilitation • Strategies • Alternatives • Education
Augmentative Alternative Communication (AAC) • What is AAC and who are candidates? • Basic • Low tech • High tech • How do we assess? • Language assessment results • Screening for candidate • Consider all factors • What happens next? • Recommend device for alternative communication device • Intervention
