The Children’s Treatment Network of Simcoe-York • Working Together for Kids & Teens with Disabilities • What about the Francophone kids and teens?
Partnership • To better serve the francophone children with multiple needs, • CEO’s of La Clé d’la Baie en Huronie and The Children’s Treatment Network • Created a unique position in July of 2007: A francophone service coordinator / team facilitator which will be serving the francophone families in French.
Major responsibilities A. Team Facilitator • Developping the unique team of professional that are francophone, fluently bilingual and can serve the francophone children with multiple needs in French. B. Francophone Service Coordinator • Coordinate services for francophone Children with multiple needs.
The Unique Team • Francophone network within the Children’s Treatment Network. • Will eventually serve the francophone children with multiple needs in their mother tongue, which is French.
First year • 7 expected outcomes were elaborated in a working plan.
ExpectedOutcome1 • Improve coordination of service operations throughout the network.
Activities • Sit on Service Coordination Task Force • Attend Single Plan of Care Training • Add a tab to Share-Point Francophone Team member • Sit on Local Team facilitator Task Force • Participate in discussions for each sites. • Sit on Reference Group
ExpectedOutcome2 • Better knowledge of French Language Services available within CTN
Activities • Design and distribute a survey to cense the Francophone capacity and resources within the various partner agencies • Raise the Reference Group’s awareness of the need to identify French Language Services within CTN
Expected Outcome 3 • Increase knowledge about services provided to children with multiple needs.
Activities • Attend workshop; Inclusive Recreation programs • Participate; Autism Spectrum Disorder Workshops • All networking opportunities.
Expected Outcome 4 • Increase identification of Francophones children with multiple needs in Simcoe/York
Activities • Developping a survey specifically aimed at schools. • Talking with the Francophone school boards superintendants and CTN directors are ongoing.
Expected Outcome 5 • Increase information mechanisms or opportunities available to promote the services offered by CTN and existing francophone coordination.
Activities • Meet with parents that attend the Ontario Early Year Centres • Meet with, Francophone Coordinator of The Ontario Early Years Centres
Expected Outcome 6 • Increase number of support mechanisms available in French for families with a child with multiple needs.
Activities • Gathering information on programs such as Triple P and Parent as Partners and so on.
Expected Outcome 7 • Increase cooperation between agencies and francophone school environments in Simcoe County / The region of York
Activities • Service coordination for 6 children, that attend all francophone schools in The County of Simcoe and The Region of York.
