TPBC Plan Template(Appendix A DES specification) • Named Clinical Lead • Agreed scope of services covered by indicative budget + links to LDP • Description of areas for redesign or plan to reduce hospital usage • Agreed baseline of referrals • Objectives to trigger component 2 • Odd quality assurance phrase

  6. PCT Tasks (1) • Benchmarking • Referral Rates • Admission Rates • First OPD Rates • OPD Follow up Rates • Supply PCT Strategic Plans

PCT Tasks (2)Activity and Financial Data • Elective Data (IP and Day Case) • Non-Elective Admissions (inc. length of stay) • First OPD and Follow ups • Use of diagnostic tests • Consultant to Consultant referrals • A&E and WIC Attendances • Prescribing • Community and Mental Health Services • GMS/PMS costs

  8. Component Two • Paid if plan objectives achieved • 95p or 70% savings whichever is the larger to reinvest in patient care, premises included
