  3. Prevention Employees can take advantage of the numerous safety training classes available through Risk Management & Safety, as well as consultative services for safety related issues Check out the RM&S website for related training @ http://www.csusm.edu/rms/ Make SAFETYyour first concern!

  4. In Case of an Illness or Injury…What do I do? Call University Police at x4567 (911 from a campus phone) An officer will come out to take an incident report and assess the situation. Paramedics will be called if necessary. The employee’s manager (MPP/Dean) will notify Risk Management & Safety at x4502.

  5. Medical Care If the illness or injury requires medical care but is not an emergency, employee will be referred by RMS to one of the campus’ occupational medical providers: Concentra Medical Center 740 Nordahl Road, Suite 117 San Marcos, CA 92069 (760) 432-9000 Kaiser On-the–Job Occupational Health 400 Craven Road San Marcos, CA 92078 (760) 510-5350

  6. Medical Care Cont’d Remember… The MPP/Dean must coordinate transportation for the employee if the employee is physically unable to transport themselves. Risk Management & Safety will call ahead to let the occupational medical provider know the employee is coming and provide authorization for the employee’s initial treatment.

  7. Recordkeeping Risk Management & Safety will provide the employee with DWC-1 and intake forms to complete and return to RM&S. Manager/Dean will complete an IIPP Form-5 (Initial Investigation Report) and return to RM&S. **These forms are available on the RMS website and must be returned within 24 hours**

  8. Missed Time Employee must use personal accruals for medical appointments. Employee may request reimbursement from the third party administrator for mileage to/from approved medical appointments.

  9. Industrial Disability Leave In some cases, an employee may be placed on Industrial Disability Leave (IDL). IDL can cover an employee for up to 52 weeks for a work related injury or illness.

  10. Return To Work In some instances, employees may need modified duty when they return to work. RM&S will assist the manager or Dean in determining duties appropriate to the restrictions recommended by the physician.

  11. Return To Work Continued Upon returning to work with modified duties, the employee may be enrolled in a Transitional Employment Plan (TEP). Each TEP is reviewed weekly up to a maximum of eight weeks.
