  1. DHAP Funding Provided: HAP Administrative Fees Case Management Fees Placement Fees

  2. HAP FUNDING: Funded Quarterly Based on DIS Leasing Information Paid Monthly beginning December 1, 2007

  3. Administrative Fee Funding Entire Fee Paid Upfront Based 15% of the “initial” monthly FEMA HAP amount for 16 Months Assume 2% Attrition Rate per month

  4. Case Management Fee Funding $92 per Family per Month Transitional Fee for September and October Regular Case Management Funding beginning November 1, 2007 Funding provided quarterly based on number families participating CM at the end of the proceeding quarter

  5. Placement Fee Funding $1000 per family placed in permanent housing To earn the fee, leasing information must be entered in DIS, no exceptions
