What is a cost report? • A cost report is somewhat similar to a “tax return”. • Cost reports are required in order to report to Medicare and Medicaid the cost of providing services to patients. • Cost reports are filed annually and provide extensive detail regarding how much money a hospital spent as compared to how much the hospital was paid.

Medicare Swing-Bed Cost Reporting
SNF PPS Swing-Beds • The SNF PPS swing-bed reimbursement methodology applies to rural hospitals that are not considered CAHs. • There are NO cost report settlements related to the swing-bed program in these hospitals.
CAH Swing Bed Services • There are cost report settlements related to CAH swing-bed services. • CAH swing-bed services are paid at 101% of reasonable cost. • Reasonable cost is determined annually from the Medicare cost report.
Swing Beds – CAH Cost Report • Days are counted based on the level of care, not the specific bed the patient occupies. • Example: A specific bed can be used for a given number of IP routine days, swing bed skilled days, and nursing facility days during the same reporting period. Therefore, all SB SNF days go on S-3 line 3, SB NF days line 4, and IP routine days line 1
Swing Beds – CAH Cost Report • The cost report will be used to determine the “cost” of providing swing-bed services to Medicare patients. • This cost will be increased by 1%. • The 101% cost amount will then be compared to the per diem rates that were received during the year by the hospital. • If underpaid, the hospital will receive additional monies. If overpaid, the hospital must pay back monies to Medicare.
Medicaid Cost Reports • Georgia Medicaid pays a fixed per-diem rate for swing-bed services. • These fixed rates are based upon prior year state-wide nursing home cost reports, NOT hospital cost reports. • There is no Swing-Bed services cost report, therefore there will not be a year-end cost report settlement for swing-bed services.
