I’m Awesome! Background • 16 year-old male • Varsity Football • Quarterback • Previous injury of growth plate separation in medial epicondyle of right elbow
Mechanism • Athlete fell directly onto L elbow • Reported hearing a “crack” in L shoulder • Immediate pain in ant. and post. shoulder CRACK!
Observation • Walked off field with arm in cradled position • Reported numbness and tingling in digits • Reported radiating pain in arm/digits
Observation con’t • Swelling was apparent below clavicle (lateral aspect) • Tenderness of the • AC Joint, • Acromion process • Deep glenohumeral joint
Special Tests • Bump test to elbow • producing an upward shear was (+) for pain • All other special tests contraindicative because of pain
Differential Diagnosis • Labral tear • AC Joint Sprain • Rotator cuff impingement
Treatment • Ice • Placed in sling • Sent for X-rays
The Results • The x-rays revealed 2 closed fractures • Upper end of humerus • Acromion • AC Joint separation (closed) WOW
Treatment post-hospital • Prescribed Vicodin and Acetaminophen • Instructed to ice and rest Goodnight….
Uniqueness • Mechanism created combination of injuries. • Risk for a growth plate fracture seeing as he is still young. • History of growth plate separation in his right elbow • The present fracture in the humerus is extremely close to the growth plate but does not go through it, fortunately
Conclusion • Continue treatments of medication, ice, and rest until the injury is fully healed. • Then begin progressive rehabilitation.
