After the initial screening or assessment, family service workers can make internal referrals and external referrals, and track the status of the referrals. 5. Process to Outcome • - The POP Mandates Portal Helps health managers and family service workers to easily identify upcoming and past due items required by the Federal and state governments. Collaboration and referrals can be made for a child. Follow up reminders can be set to remind health managers and family service workers to take further actions at a future time point.
