(281) 366-2142 • Actions: • Drilling contractor will develop a procedure to safely install the load ring onto CRT from the ground for approval by Tesco prior to utilization. • Drilling contractor should get stingers installed & operational for lift operations & provide training on use of stinger. • BP & drilling contractor management to develop a structured program for pre-tower meetings to ensure an effective CoW for all tasks. • Drilling contractor to conduct training to close gaps on: forklift specs, hand signals, risk assessment & identification. • Drilling contractor will ensure any changes made will be implemented on all other rigs operated for BP in Wyoming. 3 • What went wrong? • The IP installing the load collar on the 41/2” casing running tool (CRT) was conducting work under the elevated portion of the forklift. • Forklift operator did not stop the job when the IP began to work under the elevated portion of the forklift. • Conducting work in a congested area resulted in the IP being in an unsafe position. • Incorrect use of forklift lifting chain on single fork.
