School in CommunityOBJECTIVES • Identify and enhance practices which connect programs and services for children, families, schools and communities. • Develop linkages between school and these multiple partners to provide access to resources and information that can enhance the outcomes of the transition process. • Create traditional, as well as nontraditional alliances with community groups and organizations in order to fully expand options available.

  7. Partnerships: Key Questions • Are you aware of and do you communicate with other community partners? • Are intentional links established to community partners? How do these linkages work? • How are resources for children and families shared between community partners. • How are resources that are needed in community promoted.

Revisit your Understand your Local Community Handout and identify one stakeholder you will reach out to over the next few weeks. Be prepared to share what your experience at the next section. 2. Review your prioritized areas for Management of Transition Practices and School in Community and develop a plan for moving forward. Remember resources are available in the Transition toolkit to assist in the planning process. 3. Identify resources that may be missing from the toolkit. Be prepared to share resources that could fill these gap. 12
