HIV Risk Reduction Behavioral Intervention for Commercial Sex Workers in Armenia Implementing organizations USA PI: Ralph DiClemente, PhD Center for AIDS Research, Emory University Armenia PI: Karine Markosyan, PhD, MPH Center for Health Services Research, American University of Armenia
ARMENIA Area: 29,800 sq km Population: 3 million People: Caucasian Religion: Christian Government: Republic GDP per capita: US$884 % Population below poverty/ food poverty lines -43%/13%
HIV/AIDS Epidemic in Armenia • 410 HIV cases have been registered in Armenia from the beginning of the epidemic • Estimated HIV prevalence rate in 2002 was <0.1%
Rationale for Targeting the Project at CSWs • The HIV epidemic in Armenia is concentrated in high-risk groups including CSWs • HIV Prevalence among CSWs is around 3% • Sex workers can significantly contribute to the spread of infection to the general population
The Findings of the Study on HIV-Risk Factors of Armenian CSWs • Inconsistent condom use • Condom tear/slippage • Alcohol and drug use • Sex with injection drug users • Prominent misconceptions regarding HIV transmission, prevention and disease manifestation
Correlates of Condom Use among Armenian CSWs HIV/AIDS Knowledge 8%, p=ns Attitudes for Condom Use Intentions to Use Condoms Condom Use Behaviors 49%, p<.001 52%, p=.003 Personal Risk Perception 44%, p=.03 Comfort with Condom Negotiation 52%, p<.001
Theoretical Framework of the Intervention Module • Social Cognitive Theory • Information-Motivation-Behavioral Skills Model • Health Belief Model • Theory of Gender and Power
Project Goal Favorably change in Armenian CSWs HIV-risk and protective behaviors (with condom use as a primary focus) through the provision of appropriate information and the development of their motivation and capacity to maintain or adopt law-risk/protective behaviors.
Evaluation Design R Experimental Condition O1 X O2O3 R Control Condition O1 O2O3 O1 - baseline data collection X - intervention (HIV-risk reduction module) O2 - three-months follow-up data collection O3 - nine months follow-up data collection R - random assignment of participants to conditions
Anticipated Outcomes • Decrease of HIV-risk practices (particularly unprotected sexual practices) among CSWs • Reduction of HIV transmission among CSWs, their clients as well as clients’ female sexual partners (e.g., wives and girlfriends)
Project Significance • May contribute to the prevention of the spread of HIV in Armenia • Will build on the previous research in designing effective HIV-risk reduction behavioral interventions • May improve the basis of HIV-related science in both, Armenia and U.S.A. by human capacity building
