Possible sources: • Quality of Life Scale (PedsQL) • Warwick Child Health Morbidity Profile • Pain and Symptom Scales • School Attendance (Florida Waiver) • Impact of Family Scales (Stein and Jessup) • Family Functioning assessment • Economic Impact on Family • Provider Satisfaction Survey / NHPCO Family Satisfaction Survey

Outcome Data Evaluation Subcommittee Identify methods to access the impact of enhanced therapies such as Child Life and Art Therapy Federal Waiver Reporting Requirements # of CCS Clients potentially eligible for the Waiver # of CCS Clients offered enrollment into the pilot project # of CCS Clients who were offered enrollment and accepted Diagnoses of Clients including ICD-9 codes Paid Claims for waiver services
