INTRODUCTION : The National Rural Health Mission (NRHM) was launched to strengthen the Rural Public Health System seeking to provide effective health care to the rural populace throughout the country. Towards the end, the Indian Public Health Standards (IPHS) for Sub- Centers , Primary Health Centers (PHCs), Community Health Centers (CHCs), Sub district and District Hospitals were published. HEALTH SUB-CENTRES: In the public sector, a Health Sub-centre is the most peripheral and first point of contact between the primary health care system and the community. It is the lowest rung of a referral pyramid of health facilities. Its purpose is largely preventive and promotive but it also provides a basic level of curative care. As per the population norms there shall be one sub centre established for every 5000 population in plain areas and of every 3000 population in hilly/tribal/desert areas. PRIMARY HEALTH CENTRES: Primary Health Centre is the cornerstone of rural health services- a first port of call to a qualified doctor of the public sector in rural areas for sick and those who directly report or referred from Sub-Centres for curative, preventive and promotive health care. A typical Primary Health Centre covers a population of 20,000 in hilly, tribal or difficult areas and 30,000. COMMUNITY HEALTH CENTRES: The secondary level of health care essentially includes Community Health Care Centers (CHCs). The CHCs were designed to referral health care for cases from the Primary Health Centre's level and for cases in need of specialist care approaching the centre directly. 4 PHCs are included under each CHC thus catering to 80,000 populations in tribal/hilly/desert areas and 1,20,000 population for plain areas. HEALTH CENTERS – A CASE STUDY • Existing health centers were studied to suggest betterment. As per the IPHS guidelines and existing conditions, some suggestions are proposed for the up gradation and betterment of the health centers. • KEY POINTS PROPOSED : • USE OF SOLAR PANELS • PROPER SANITATION • PROVISION OF SEWAGE TREATMENT PLANT SUB CENTER - These centers are for the Abhanpur District. Sub center and Primary health center opens every day but for a limited hours. And community health center is open for 24 hrs. SANITATION EXISTING TOILET The existing toilet lacks an efficient flushing system resulting in unhygienic conditions. Flushing cisterns require maintenance and are also have high cost of installation. Also, the exposed pipelines have a danger of vandalism. PROPOSED METRO FLUSH Metro flush is proposed for the toilets. The water supply pipeline is of 1.5 inch thick and water flows directly from an over head tank through a concealed pipeline. The flow is regulated by a valve located approximately 15 inches above finished floor of the toilet, This type of system is used as it is economical in installation and also requires low maintenance. HEALTH CENTERS – A CASE STUDY EXISTING TOILET The existing Toilet does not have a proper drainage point for waste water. The puncture in the wall adversely affects hygiene conditions by allowing insects, germs and dirt to enter the bathroom. Also, wooden doors tend to rot after some years. PROPOSED TOILET • Proposed bathroom is to have a nahani trap for collection and carrying of waste water to the nearest drain line. A cover with grating is provided at its top so as to prevent the entry of solid matter. • PVC doors are suitable for the bathroom as they are durable under wet conditions. SOLAR PANELS - STUDY In the modern times, energy saving is an important issue. Buildings are the major consumer in energy sector. So alternative energy sources are needed. At Chhattisgarh, the major renewable resource of energy available in abundance is sunlight. So we have proposed SOLAR PHOTOVOLTAIC PANELS as the alternative source of energy. Solar PV panels works on about 25% efficiency considering 12 hours of daytime out of which about only 4 hours of peak time. • A single panel of solar pv is of size 1.66m x 1.0m. To generate 1 kW of energy , 4 panels are to be installed. • Cost of Installing 1 kWh is Rs. 1.50,000/- and the Subsidiary by the government is 90,000/-. • Benefits • In case of Blackout, solar power enables the working of basic • equipments. • Also, water supply to the overhead tank can also be ensured. • Excess power generated can be transferred to the grid. • less maintenance Designing Models For Rural Toilets And Govt. SOLAR PANELS – IMPLEMENTATION IN SUB-CENTRES Considering usage and feasibility of Solar Panels, they are proposed at health sub centers. Electricity generated through solar panels shall be used for running of pumps to ensure the uninterrupted supply of water. For Health Sub Center – 1 Open well Pump of 1.0 hp requires 750 watts per hour. For day to day water consumption, average 2 Hours running of pump is required. (1 unit of Electricity = 1000 watt) That means 2 kW OR 2 UNIT of electricity is required per day for continuous water supply. For Solar Panels : With 25% efficiency, peak hours = 4 hours Capacity of solar panels = 1 kWh Total energy produced per day = 4 kW or 4units of Electricity Hence, by installing 1 kW solar Panels, continuous water supply can be assured. TOILET LAYOUT: SUB- CENTRES W.C TOILET BATH TOILET TYPICAL PLAN OF SUB-CENTRE • The location of toilets in a typical sub centre is shown. • Soil pipes of 100mm dia. from W.Cs and waste water pipes of 65mm dia. from nahani traps in bath areas to be provided. • Inspection chambers are proposed at junctions of the pipelines, such that the maximum distance between them is 10 feet or 3.0m. • The drain line from any adjoining rooms (labor room or sterilization room) shall be connected to the nearest nahani trap. • The soil pipe shall lead to a septic tank located on the site while waste water pipe towards the drain line. • The septic tank is to be located according to the natural slope of the site and also in such a way which ensures minimum length of pipelines. • The pipes must be laid in a slope of ¼” per feet or 20mm per meter. • Water supply shall be from an Over Head tank for which a sump has to be installed. TOILET LAYOUT: SUB- CENTRES TYPICAL PLAN OF A TOILET • Guidelines for construction of toilet: • The W.C must be installed on a raised platform (100mm to 200mm) to accommodate the P-trap as shown in Section ‘bb’. • A drop of 10mm in the finished floor level at the entrance of toilet. • The nahani trap is to be located in opposite direction of toilet entrance. • Anti Skid tile flooring shall be done to avoid accidents. • The slope must converge towards the W.C pan. • Concealed pipelines are to be installed to avoid vandalism and damage by corrosion. TOILET LAYOUT: SUB- CENTRES NAHANI TRAP • Guidelines for construction of toilet: • The tile flooring must be laid after a layer of P.C.C on cinder filling or filling with construction debris. • The slope must be provided at 1:100 to converge towards the nahani trap. • The Pillar must be provided at the height of 1.0m as shown in the diagram. • Dado wall with ceramic glazes tiles upto lintel level to ensure easy cleaning and less water retention. TOILET LAYOUT: SUB- CENTRES • Guidelines for construction of toilet: • The W.C pan is to be placed with the minimum setback from the wall as shown in the plan such that the bib cock is on the right hand side of the pan orientation. • The slope of the tile flooring must converge towards the pan at 1:100. • Bib cock and metro flush valve must be located on the wall as shown in Section bb. • Inspection Chamber • The inspection chambers are located at junction of pipelines. For Solar Panels : With 25% efficiency, peak hours = 4 hours Capacity of solar panels = 3 kWh Total energy produced per day = 12 kW or 12 units of Electricity Hence, by installing 3 kW solar Panels, water supply can be assured.
