ANTIPROGESTIN Antagonizes or suppresses the action of progesterone Role of progesterone: Maintain pregnancy Favouring pregnancy Secretory changes
SELECTIVE PRGESTERONE RECEPTOR MODULATOR • MIFEPRISTONE (Partial agonist) 2) AGLEPRISTONE (full agonist) 3) ONAPRISTONE (pure progesterone antagonist) 4) ULIPRISTAL (partial agonist on progesterone receptor) 5) ASOPRISNIL (investigational SPRM)
MIFEPRISTONE • 19-norsteroid • Potent antiprogestational • With significant: • Antiglucocorticoid • Antiandrogenic activity
MIFEPRISTONE If given during the follicular phase: • Attenuation of the mid cycle Gn surge from pituitary • Slowing of follicular development • Delay/failure of ovulation If given during luteal phase: • Prevents secretory changes by blocking progesterone action If given after implantation: • It blocks decidualization • Dislodging of Conceptus • Fall in HCG production • Abortion
MTP BY MIFEPRISTONE • Nonsurgical method of abortion • Up to 7 weeks of pregnancy: • Within 10 days of a missed period • 200 mg as single oral dose • Followed by 400 mg oral misoprostol
USES OF MIFEPRISTONE • Termination of pregnancy • Cervical ripening • Postcoital contraceptive • Once a month contraceptive • Induction of labour • Cushing’s syndrome (glucocorticoid receptor blocking property)
