Brief History of the Team • Disability Action was set up in 1990 by Dr Chris Moran, as a research project based around 6 GP surgeries. • The service is now jointly funded by NHS Dorset and NHS Bournemouth & Poole. • Disability Action has evolved into an inter-disciplinary team providing a single point of contact, specialist assessment, on-going monitoring, and support for clients with long-term neurological conditions. • The team covers the former South & East Dorset, Bournemouth and Poole areas.
Consultants • Dr. John Burn, Consultant in Rehabilitation and Brain Injury. • Dr. Emma Murphy, Consultant in Rehabilitation Medicine. • Dr. Claire Myint, Consultant in Rehabilitation Medicine, Orthotics and Wheelchair Service. Also have Strong Links with: • Dr. Charles Hillier, Consultant Neurologist with Special Interest in Neuro-Inflammatory Disease.
Client Group and Criteria • Adults (18 years and over). • Neurological/Neuromuscular diagnosis which leads to physical disability. • E.g. Multiple Sclerosis; Huntington’s Disease; Cerbral Palsy; Muscular Dystrophy • Physical disability and related diagnosis is primary problem. • E.g. Not Mental health diagnosis; drug & alcoholism • There is not another similar team or professional managing that patient. • E.g. The Poole MS Team; Parkinson’s Disease Specialist Nurse
Disability Action Process • Single point of access for people with a long-term neurological condition • Thorough inter-disciplinary assessment and reviews • Access to specialist neurological professional assessment and advice • Regular contact/support from the team as best meets clients’ needs (not time limited)
MDT support of specialist clinics • Inter-agency working • Access to support workers as required • Telephone access to team 5 days per week • Signposting to relevant services/agencies • SOS system available as indicated
Joint Working Disability Action works closely with colleagues in both statutory and voluntary sectors

Consultant Rehabilitation Clinics • Dr Emma Murphy • Dr John Burn • Huntington’s Disease Clinic • Evolving inter-agency & MDT clinic involving Consultant, Nurse Specialist, HDA, Genetics, Dentist, Clinical Neuro-Psychology, (DA Therapy) • MS Nurse Led Clinic • Held at Osbourne Day Centre (MS Society) bi-weekly
CP Clinic • Disability Action Specialist Nurse and Therapists will input into the proposed MDT CP clinic • Awaiting consultant lead • Social Health & Care • Joint assessment & review • Joint care planning • Compilation of joint reports as required
Developments • Plan in place for reducing waiting time for initial assessment to 2 weeks by the Summer 2010 • Have developed improved standards for minimum contact • Now able to offer specialist neurological nurse, physiotherapy and OT assessment and advice to all caseload • Have started to jointly develop services for HD • Have started to look at service improvement for clients with Muscular & Myotonic Dystrophy
Are ready to input into MDT CP Clinic • Continue to look at viability of ‘one-stop’ MDT clinics throughout catchment area • SOS process developed • Due to start reviewing Process Map with service users and other interested parties (Meeting August 2009) • Process pathways and database currently being reviewed and updated to improve client care, data collection and facilitate audit • Have started to review standard and outcome measures to best suit client groups and improve quality of data for audit • Team launch to be arranged to improve awareness in primary and secondary care
