File Maintenance File/Clinical/Configure Agency Templates Select Performance Measures Performance System and Performance Measurement are CBN controlled Lots of measures in Vital Signs More will be added with time (enhancement requests!) Select Diagnosis- optional Can select many diagnosis Selected diagnosis do not have to be in one range Best Practice Protocol Can link document Relative path means it is in the HW drive Templates are also configurable in the Dashboard in HW and RN Two Kinds of Templates • Diagnosis-Specific • Graph will only display for patients with the selected diagnosis • Non-Diagnosis Specific • Graph will display for ALL patients Configuring Agency Templates
Documenting in Outlines Visit Note Outlines • Data is entered in a profile outline attached to a visit • Be careful of custom outlines! • Custom outline must use our Performance Measures in order to graph • Profile outline does not have to be completed or locked

Three Main Screens • Find Patient • Patient Overview • Patient View Graphing in the Dashboard Patient Overview • Displays graphs for patients • Based on template • Template could be based on diagnosis • Can add patients w/o the diagnosis • What you can do: • Uncheck the checkmark to remove from graph • Right-click menu includes adding a patient, removing this patient, configuring the patient color, and viewing contacts • Same functions available at the bottom • Template Properties button- with template highlighted can view best practices and print

Three Main Screens • Find Patient • Patient Overview • Patient View Graphing in the Dashboard Patient View • What you will see: • Length of Stay • Reimbursors • Last and next scheduled visits (limited to 3) • Active Orders • What you can do: • Switch to Cost/Revenue or Authorizations • Template Properties- view/add contacts • Switch back to graph
Using the Dashboard in IDT Meetings Tips on Using the Dashboard to Document IDT Meetings • Have team secretary configure dashboard to list all the patients assigned to that team. • HW and Dashboard is projected onto wall during the meeting. • The first template on the patient overview screen could be as simple as pain level. • Double-click on each patient as he/she is discussed. • Template for that patient can display additional clinical measurements per your agency's preference • Quick review of DX, schedule, Orders, Care plan. • Add a contact with service code = "TEAM MEETING", attach a profile to document the discussion and who was present, etc • Send a secure message to team members not present. • Click on next patient in list and repeat. • Note: by having CA open as well and narrowed down with the FIND PATIENT screen to active patients on census & that team, they can easily toggle between CA and Dashboard to make modifications to care plan or orders.
