  5. Home visits Peripatetic team Visit within 6 months of discharge Skin problems/pressure sores Case conferences OT/Physio related issues Involve community staff

  6. However We are not an emergency service! • First line treatment is through the GP, District nurse or local hospital • Home visits and outpatient visits must be arranged by appointment • After initial discharge, entitled to transport ONLY for first discharge appoint.
