This research study aims to enhance the role of non-GP staff in chronic disease management in general practice through team building, training, and clear goal-setting. The study will investigate how enhanced teamwork can lead to improved care for patients with diabetes, ischaemic heart disease/hypertension, increased patient satisfaction, and increased job satisfaction for staff.
